What is SystmOne/SystmOnline and how to register

You can use our online services to

• Make or cancel appointments
• Request repeat prescriptions
• Review your medication
• View your test results
• Change your contact details

You will need to print off the form (page 2 of the document) and bring into the surgery with proof of your identity.

Please bring your completed registration form to The Surgery to register and collect your login details. All patients registering for this service will need to provide photographic identification, e.g. passport, student card, bus pass, driving licence in order to be issued with a user login. For security and confidentiality reasons we can only give logins to the individual in person and we only offer the service for patients aged 13 and over. Login information will not be posted, emailed or faxed to anyone and will not be given out over the telephone. Login information will not be given to parents/guardians of young people or any other third parties.
