Use this service to register with a South Coast Medical Group Surgery. We have 5 Surgeries covering most of the Bournemouth and Christchurch area. You may wish to choose the Surgery closest to your home address.
Information:
We provide a home-visit service to housebound patients who live within their Surgery’s practice boundary. You can still register with us if you live outside your chosen Surgery’s boundary, but you will not be eligible for this service.

Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
You can also visit the surgery you wish to register with and pick up a registration pack in person.
